Polymorphic rash of pregnancy (a.k.a., Pruritic urticarial papules and plaques of pregnancy; PUPPP). It is one of the most common skin conditions to occur during pregnancy… and it more often affects women who have never given birth, occurring sometime between the last few weeks of pregnancy to the immediate postpartum period. However, there are exceptions, with some women being affected earlier on in pregnancy. It is a benign, pruritic skin disorder characterized by a papules and plaques, often occurring within striae. Other lesions can develop as well, including target-like lesions, giving it a polymorphic appearance. But in simpler terms, it’s a bumpy and itchy rash. Now it can cause extreme itchiness, and have an unpleasant appearance, but other than that, it does not pose any long-term risks to either the mother or child. This rash usually beings on the abdomen and then spreads to the limbs, chest, and neck. There is usually sparing of the periumbilical area, face, palms, and soles. The diagnosis is made clinically. Specific testing or a skin biopsy are not usually needed. Initial treatment of is usually conservative, with the goal of symptom relief. Topical corticosteroids are recommended as initial therapy. The rash and itchiness usually disappears within a few weeks, even without treatment. The most important aspect to remember about this condition is that despite being annoying, it is benign, limited in duration, and is not otherwise harmful to either mother or child. As well, recurrence of the rash in subsequent pregnancies is uncommon.
