Atrophoderma Vermiculatum (AV)



Atrophoderma vermiculatum is a rare, chronic, follicular keratinization disorder of the skin, characterized by atrophic, reticulated, honeycomb-like, or worm-eaten (vermicular) depressions. Pic






Etiology(causes)?

The exact cause is unknown.

Possible contributing factors:
• Genetic predisposition ( autosomal dominant).




Clinical Features

• Onset: Childhood below 10 years of age, slowly progressive for several years, then stabilizes.

•Apprearance:

• Symmetrical, pit-like depressions with a reticulated (“honeycomb”) pattern.
• Usually skin-colored or slightly hypopigmented.

• Distribution:
• Most common: Cheeks (malar area).
• Less common: Forehead, temples, preauricular area.

• Symptoms: Usually asymptomatic; cosmetic concern is main issue.

• Course: Gradual progression → plateau → persists lifelong.



Association with
• Keratosis pilaris is common.

Diagnosis

• Clinical appearance is usually diagnostic.


Similar looking skin conditions?
1) Acne scar
2) Follicular atrophoderma
2) chickenpox scar

Treatment

Unfortunately, there is no definitive cure; focus is on cosmetic improvement.

Options:
1. Topical keratolytics:
• Salicylic acid, urea, lactic acid.
• May smooth surrounding skin.
2. Topical retinoids (tretinoin, adapalene):
• Reduce follicular plugging.
3. Oral retinoids (isotretinoin, acitretin):
• Tried in progressive cases; variable results.
4. Dermabrasion / Microdermabrasion:
• Can improve texture.
5. Fractional laser (CO₂ fractional):
• May stimulate collagen remodeling and smooth surface.

Prognosis
• Lesions do not resolve spontaneously.
• Cosmetic impact is the main concern.
• With cosmetic interventions, texture can be improved, but complete reversal is rare.
