A patient with prior TAVR (29 mm CoreValve) and stenting of the proximal LAD x 2 presented with unstable angina. Coronary angiography demonstrated severe distal left main, proximal circumflex, and proximal LAD (in-stent) disease. An IABP was placed. He was turned down for CABG and referred for complex PCI. Engagement of the left main failed via right radial access but was successful through left radial access with a 6 French JL4 guide catheter. IVUS showed good stent expansion with neointima formation. After predilation the left main bifurcation was stented using the DK crush technique with an excellent final result and no complications. The circumflex was considered as the main vessel and the LAD as the side branch as rewiring the LAD was easier.
