1) What exactly happened and why should we be concerned about it?
Change Healthcare (one of the largest prescription processors in the U.S.) was affected by a cyber attack from a suspected nation-state actor. This affected nationwide hospitals, pharmacies and U.S. military clinics across the world. All partners affected by this disruption were advised to get disconnected from Optum. It’s been 5 days and the situation has not been resolved. CH is touching 1 in 3 patients which is overwhelming. In 2024, a network being down for so many days is unimaginable.

2) How has it impacted the U.S.?
a) It directly impacts the filling of prescriptions for patients that need life saving medications like insulin and cancer related drugs. With insurance verification delayed or down, filling prescriptions are having massive delays, it seems some patients are being asked to pay for the medications up front and then submit a reimbursement request to the payer when things start to get back to normal. This is an extreme burden put on the patient for something that was not caused by them.

b) It seems second most immediate would be that patients may not be able to see their doctor or have a medical procedure if their insurance coverage cannot be verified or authorized. Eligibility verification and Service Authorization are critical steps in the healthcare process when patients are seeing a new doctor or specialist for the first time or having a needed medical procedure.

c) It seems next would be the worsening apprehension of hospital and clinic c-suite administration because in today's healthcare market, revenue cycles are thin and play a big role in future strategic plans to take care of the patient base. A small neighborhood clinic often depends on the payment from the state Medicaid system to make payroll for the coming weeks. This issue will begin to snowball. We are not sure how easily prescriptions can be re-routed.

3) How does it directly impact us as EDI professionals?
All of a sudden, we have to scramble for alternatives. We need to have a plan B and be proactive. Hoping and wishing is not a good strategy. We can always use direct connections which most people are not aware of.

4) How do we circumvent a situation like this?
We could keep patches up-to-date but when a conglomerate is likely targeted by a nation state actor, it brings defensive security to the next level. The problem is we don't even know where the issue is in order to fix it. Peter's company provides alternative solutions to such situations like establishing a direct connection, re-routing through another provider etc.
