This is a screen capture of an INP file that my Step Dad submitted. In the course of his play, he performed the correct set of actions to get the glitch. It seems that this glitch can be performed over and over again as an INP file is nothing more than a recording of inputs which are then played on MAME.

Please note that WolfMAME version 201 was used to record this INP file. My WolfMAME build was compiled from Source on Ubuntu MATE Linux 18.04 LTS.
