Treatment for Otitis Media focuses on clearing the infection in the middle ear and relieving pressure behind the eardrum. Most children and adults start with oral antibiotics, which usually help within a few days. If there is no improvement after completing the course, the next step is IV (intravenous) antibiotics given in the hospital. Improvement is usually seen within 24–48 hours.

When the infection remains severe, especially if the eardrum is bulging and close to bursting then a small procedure is needed. A tiny cut is made on the eardrum and a ventilation tube (grommet) is inserted. This tube helps equalise pressure, drain fluid, and restore normal middle-ear function.

The tube is very small and will fall out on its own once it has done its job. The eardrum then seals naturally without leaving a hole. Many children wake from anaesthesia saying, “I can hear again!” because the pressure and pain are relieved immediately.

Some children also have large adenoids blocking the Eustachian tube. In those cases, both the grommet procedure and adenoid treatment are needed to prevent recurrence. Adults may require additional treatment if sinusitis, nasal polyps, or in rare cases, nasopharyngeal carcinoma are contributing to the problem.
