This animation demonstrates the steps to performing a bilateral breast reconstruction with pedicle TRAM flaps. It opens with a view of the torso of an obese woman with mastectomy scars and the rectus abdominus muscles visible. The scars are removed. A long abdominal incision is made above the umbilicus. The path of two tunnels under the skin up to the mastectomy incisions is shown. The remaining abdominal incisions are made to create TRAM flaps. The right and left flaps are transected and vessels are clamped. The flaps are pulled under the skin to the mastectomy incisions. The abdominal incision is closed and drains are placed. TRAM flaps are pulled through mastectomy incisions and drains are placed. TRAM flaps are trimmed and sutured into place.
