Stage 4 Cancer में पेट का पानी कैसे निकालें? | Fluid Buildup in the Abdomen! | Dr Praveen Kammar
In this video, I’m discussing what treatment options we have when stage 4 colorectal cancer leads to fluid buildup in the abdomen what we call malignant ascites.
Why Does Fluid Accumulate in the Abdomen?
In stage 4 colorectal cancer, cancer spreads to the inner lining of the abdomen—called the peritoneum. This spread is known as peritoneal metastasis, and it’s the most common reason for abdominal fluid accumulation, technically called malignant ascites.
Why Aggressive Surgeries Often Don’t Work
Procedures like Cytoreductive Surgery (CRS) and HIPEC are options in some cancers but are rarely feasible in advanced colorectal cancer with extensive peritoneal spread. We evaluate them case by case, but they are usually not suitable here.
Treatment Option 1: Tapping or Catheter Drainage
One effective way to relieve symptoms is to remove the fluid using a needle (tapping) or a permanent catheter for continuous drainage. This relieves pressure, improves breathing, digestion, and mobility. However, frequent tapping increases the risk of infection or bleeding. Catheters are safer for long-term use but need proper care.
Treatment Option 2: Diuretics
In select patients, diuretic medicines (typically used for blood pressure) may offer partial fluid control. But their effectiveness is limited in malignant ascites.
Treatment Option 3: Chemotherapy
Chemotherapy has shown excellent results in ovarian and stomach cancers with ascites. In colorectal cancer, the response is modest. Still, for some patients, it remains the best available option—30–40% may see fluid reduction or even full resolution.
Advanced Treatments: Targeted and Immunotherapy
Some patients benefit from targeted therapies like Bevacizumab or immunotherapy, depending on their cancer profile. These may help control ascites more effectively in selective cases.
New Technique: PIPAC (Pressurized Intraperitoneal Aerosol Chemotherapy)

Rare Option: Peritoneo-Venous Shunt
An older method involves connecting the peritoneal cavity to blood vessels to drain fluid back into circulation. Due to complications and limited patient tolerance, we rarely use it now.
