In this fourth masterclass on fertility, we discussed a case of a couple with a history of six abortions, four of which were spontaneous. Genetic testing revealed a normal variant in the husband's karyotyping, which could be contributing to the miscarriages. The couple underwent IVF, and a low mosaic embryo was transferred, but it did not result in pregnancy. High mosaic embryos were not transferred due to the risk of chromosomal abnormalities.

We recommended whole exome sequencing for both partners to gain a deeper understanding of their genetic structure. Based on the results, we would conduct genetic counseling and non-invasive chromosomal studies to ensure the embryo is chromosomally normal, increasing the chances of a successful pregnancy. Additionally, a thorough evaluation of the female partner’s immune system is essential to rule out immunological factors.

While sperm donation is an option, it would require genetic testing of the donor sperm as well. This case highlights the importance of genetic testing, personalized counseling, and careful decision-making in fertility treatment.
