Educational video describing supination and pronation fracture classification. Medial malleolus fractures and fixation.
Supination-adduction
10-20% of malleolar fractures
Medial displacement of the talus (only type).
Stage 1: transverse avulsion of fibula distal to the level of the joint or rupture of lateral collateral ligament.
Stage 2: vertical medial malleolus fracture associated with injury to talus and tibial plafond.
Check for injury to the talar dome or tibial plafond.
Treatment
Medial malleolus
-Open reduction and internal fixation
-Inspect and clean the joint
-Check for loose bodies
-Lag screws or anti-gliding plate : lag screws parallel to the joint and perpendicular to the fracture
Fibula
-Lag screw or plate
