Sudden Sensorineural Hearing Loss (SSNHL) is a rapid, unexplained loss of hearing — usually in one ear — that occurs over a period of hours or days.
It’s considered a medical emergency.
The sooner it’s diagnosed and treated, the better the chances of recovery.
Sudden Hearing Loss?

Good hearing permits us to have a conversation with the people we love, permits us to enjoy music, and overall keeps us connected with the world around us. Losing this vital sense is catastrophic. In spite of this, I get to see many patients who ignore warning signs of a sudden loss of hearing and report late.
SUDDEN SENSORYNEURAL HEARING LOSS
In the most typical situation, the patient reports sudden loss of hearing in one ear. In very rare instances hearing loss can be in both ears. The patient reports that there is a feeling of vacuum inside the ear, there is obvious hearing loss, there may be a hollow feeling. There may be accompanying symptoms
• Tinnitus - a buzzing sound in the affected ear
• Giddiness - this indicates a broader insult to the inner ear - this symptom usually indicates a worse prognosis - the patient may not respond well to treatment.

By definition, Sudden SNHL is diagnosed when
• Over a period of three days or less
• There is a minimum 30 dB hearing loss
• In minimum three consecutive frequencies
Cause
1. In a vast majority of cases, no cause is found- the so-called Idiopathic cases. In Idiopathic cases, the cause could be a viral infection, vascular ischemia, breaks in inner ear membranes, or some autoimmune phenomenon.
2. Vestibular Schwannoma- a tumor close to the inner ear can sometimes cause a sudden onset hearing loss
3. Stroke The disease can affect anyone including children though middle-aged persons are more prone.
MANAGEMENT
The most important consideration is to visit the Specialist doctor at the earliest. Initial ENT examination and audiological evaluation will determine the diagnosis. Some tests including simple blood workup, MRI of the Brain or CT scan of the Inner ear may be asked for.
The TREATMENT consists of
• Systemic Steroids - this is the mainstay of the treatment and have to be started early - preferably within hours of onset - to get the best results
• Specific treatment for a known cause
• Intratympanic steroids - for patients who fail to respond to systemic steroids or those in whom systemic steroids cannot be given for some reason - uncontrolled diabetes, etc. The steroid is injected directly into the ear
Prognosis is dependant on many factors including the patient's age, the gap between the onset of symptoms and starting of the treatment, accompanying giddiness, audiometric configuration, comorbid conditions like diabetes, etc.
My parting message - in case you experience a sudden loss of hearing, even if you feel that the ear is blocked please visit the ENT Specialist at the earliest and at the least get an audiological evaluation done.
This will enable an early diagnosis and treatment and ensure the best possible outcome for recovery of hearing.
