Praz 0.5 mg (generic: alprazolam), a benzodiazepine:
(Uses)
Approved uses:
• Generalized Anxiety Disorder (GAD)
• Panic Disorder (with or without agoraphobia)
Off-label (short-term, doctor-prescribed use):
• Insomnia
• Anxiety linked to depression, psychosomatic disorders, PMS, PTSD
How It Works
Enhances GABA neurotransmission by binding to GABA A and benzodiazepine receptors, which calms overactive brain activity, reduces anxiety, induces sedation, muscle relaxation, and can help control seizures
Dosage Guidelines (0.5 mg tablet)
1. Typical adult: 0.25–0.5 mg every 6–8 hours; may increase cautiously (max 4 mg/day for anxiety; panic disorder doses often total 3–6 mg/day) .
2. Elderly/hepatic or renal impairment: Lower starting doses (0.25–0.5 mg once daily), titrating slowly

Side Effects
Common (usually mild):
1. Drowsiness, sedation, fatigue
2. Dizziness, light headedness, impaired coordination
3. Headache, dry mouth, GI upset (nausea, diarrhea, constipation)
4. Memory or concentration problems, increased salivation
Serious (seek medical advice):
1. Confusion, slurred speech, hallucinations, mood changes (e.g., depression or suicidal thoughts)
2. Respiratory depression, especially with other CNS depressants
3. Rare: paradoxical reactions (agitation), worsened depression, seizures on withdrawal
Contraindications & Precautions
Absolute contraindications:
1. Allergy to alprazolam or benzodiazepines
2. Acute narrow-angle glaucoma
3. Severe respiratory insufficiency or sleep apnea
4. Myasthenia gravis
5. Pregnancy (Category D) and breastfeeding – passes into breast milk
Cautions:
1. History of substance use or addiction
2. Elderly (risk of sedation, falls, cognitive impairment)
3. Hepatic or renal impairment – may require dose reduction
4. Depression or suicidal ideation – benzodiazepines may exacerbate
5. Co-administration with CYP3A4 inhibitors (e.g. ketoconazole, itraconazole, macrolide antibiotics, grapefruit juice) — may raise blood levels and toxicity
6. Alcohol, opioids, barbiturates: dangerous CNS depression
Withdrawal & Dependence
1. Risk of tolerance and dependence increases with prolonged use; abrupt cessation can cause withdrawal: anxiety, insomnia, irritability, seizures, hallucinations .
2. Always taper gradually under medical supervision.
