Postural Drainage for Middle lobe.
Postural Drainage is a intervention for airways clearance by mobilizing secretions in one or more lung segments by placing patient in various positions, so that gravity assists in drainage process.
It is effective in cystic fibrosis, bronchiectasis, and other pulmonary diseases.

• Goals and Indication-:
1.Prolonged bed test
2.Patients on ventilator
3.Painful incision causing restricted deep breathing and coughing.
4.Chronic obstructive pulmonary disease.
5. General Anesthesia
6. Acute or chronic lung disease
7. Generally weak or elderly people
8. Patient with artificial airways

• Contraindications -:
Sever hemoptysis
Pneumothorax
Pulmonary embolism
Recent Myocardial infraction
Unstable angina
Congestive heart failure
Large pleural effusion
Recent neurosurgery
Cardio arrhythmias
Sever hypertension or hypotension

• Prerequisite -:
1.Vital signs
2. Meal and medication atleast before 45 mi should be done
3. Forced expiratory technique (huffing and coughing)
4. Deep breathing exercise
5. Humidification and Nebulization should be given
6. Sputum mug provided to the patient

• Manual Techniques -:
1. Percussion
2. Vibration

• Procedure -:

1. Positioning:
The patient should lie on his back with his body quarter turned to the left maintained by a pillow or wedge maintaining 15 to 20⁰ under the right side from shoulder to hip and the arms relaxed by his side and visa versa.

2.Breathing Exercises:
Deep breathing exercises or controlled coughing may be encouraged to help loosen mucus from the airways.

3.Percussion and Vibration:
After positioning, percussion (gentle clapping) and vibration (manual shaking) are often applied to the chest wall. This helps further loosen mucus so that it can be expelled through coughing.

4.Duration:
Each position is usually held for about 5-15 minutes, depending on the patient's tolerance and the effectiveness of mucus drainage.
