You And Your Warfarin

Warfarin patient information video: This video was produced by NHS Fife Pharmacy Services. It is one of the resources developed through their involvement in the Scottish Patient Safety Programme, and was produced in collaboration with Health Improvement Scotland and NHS Chesterfield.
The video is intended to assist in the counselling of/refresh the knowledge of patients who are currently prescribed warfarin, as well as their associated carers and family members etc.
There is also a counselling tool available for pharmacists to use with a ‘QR’ code that can be scanned by smart phone / tablet technology that links to this video. This assists the pharmacist in performing High Risk Medicine Interventions and allows patients etc to re-view the video at a later date on their own device
