Phrenic Nerve:

Nerve roots: Anterior rami of C3, C4 and C5. Mainly originates from C4 spinal nerve root, but also receives contributions from C3 and C5.
Very long nerves.

Bilateral nerve that originates in cervical spine and descends through thorax to reach diaphragm.
Motor function: Diaphragm. (Main respiratory muscle.) Bilateral structure, each nerve supplies ipsilateral side of diaphragm.
Sensory function: Innervates central part of diaphragm.
