Dhanvantari Mantra:

oṁ namo bhagavate vāsudevāya dhanvantaraye
amṛta-kalaśa-hastāya sarvāmaya-vināśāya
trailokya-nāthāya śrī-mahā-viṣṇave namaḥ

We learn from Srimad Bhagavatam 2.7.21:
“The Lord in His incarnation of Dhanvantari very quickly cures the diseases of the ever-diseased living entities simply by His fame personified, and only because of Him do the demigods achieve long lives. Thus the Personality of Godhead becomes ever-glorified. He also exacted a share from the sacrifices, and it is he only who inaugurated the medical science or the knowledge of medicine in the universe.”

Below is the Saṅkalpa mantra, that was used for HH Jayapataka swamy well being. Devotees, who want their health also to be good, can keep their full name in the highlighted text in Sankalpa mantra before you start actual mantra and chant the Dhanvantari mantra for one week or two weeks or determined period. If you chant this mantra daily 108 times, always one will be healthy.

atha śrī-dhanvantari bīja-mantra japa pratidinam aṣṭottara-śata vāram aham kariṣye
iti saṅkalpaḥ
asya śrī dhanvantari bīja mantrasya
bhagavān śrī dhanvantari devata
śrīla jayapatākā svāmī (mama) gurudeva dṛḍha ārogya dīrghāyu siddhyarthe śrī dhanvantari bīja mantra jape viniyogaḥ

108 times minimum per day to chant:
oṁ namo bhagavate vāsudevāya dhanvantaraye
