Life in rural towns can be peaceful, but these communities have challenges of their own. Due to physician shortages and lack of access to care, the mortality rate in rural America is 23% higher than urban settings.

Having grown up in a small town and experienced the lack of access to care following her type 1 diabetes diagnosis, Homeward Co-Founder and CEO Dr. Jennifer Schneider witnessed the benefits and challenges of living in a small town firsthand. This inspired her to found Homeward Health. In conversation with Chris Bischoff, managing director of General Catalyst, the two will discuss Homeward’s approach, including the company’s innovative business model, care delivery, and technology to address the rural health crisis head on.
