The best position to sleep after a total knee replacement is the one that allows you the best night's sleep.

Sleeping after a total knee replacement can be the most frustrating part of recovery. The conventional advice is to sleep flat on your back with the knee in full extension. The problem is many of my clients have not been able to sleep in this position for years. Spinal stenosis, sleep apnea, and other medical conditions make sleeping in this position impossible.

Large bodies of medical research have suggested sleep is a vital component of recovery. This research suggests that one night of interrupted sleep may increase stress hormones, increase blood sugar levels, increase pain sensitivity and more.

If you are working on improving knee extension during the day and you are not experiencing significant loss of knee extension when measured by yourself or your physical therapist then it is reasonable to consider sleeping in any position that affords you the best quality sleep.
