Patterns: papillary with slit-like spaces, solid, endometriod, transitional - SET appearance more commonly associated with BRCA1 mutations -, microcystic/microglandular
Nuclear pleomorphism, more than 3x variation in size
High mitotic index: more than 12 mitotic figures per 10 high power fields, often atypical
Necrosis
Immunohistochemistry: p53+, p16+, WT1+, PAX8+
The stage is the most important prognostic factor
