The last sewer level is also the hardest, but what I found myself struggling the most with was the bonus stage, and especially the bouncy boxes at the end. Also, those flying things at the end. I could never really figure out exactly where their paths are and where you're safe, so maybe I was more careful than I had to be. Either way, I made it through and that's all that matters. Going through the bonus exit unlocks the next secret level.
