Adnan Qureshi, MD, University of Missouri-Columbia, Columbia, MO, comments on the current clinical practices of intra-arterial thrombolysis (IAT) in the era of mechanical thrombectomy. IAT is widely used in daily practice for several approaches, including the treatment of primary distal occlusions, as rescue after proximal occlusion thrombectomy, or as adjunctive therapy to primary mechanical thrombectomy. However, a lack of randomized trial evidence means there are no definitive rules on its application and insights into the risk-benefit profile in different populations is lacking. Dr Qureshi also comments on the uncertainties surrounding intravenous thrombolysis (IVT) use and if IAT could ever replace IVT in patients with acute ischemic stroke. This interview took place during the International Stroke Conference (ISC) 2023 in Dallas, TX.
