The accident at the Deepwater Horizon led to an explosion and fire that killed 11 people and caused pollution in the Gulf of Mexico. This video goes through the happenings step-by-step, while showcasing the key findings from the investigation. The investigation into the accident showed that no single factor caused the tragedy in the Macondo well, but rather a sequence of failures leading up to the accident.
