What Documents needed with the form?
1. Marks Sheet (Matric)
2. Marks Sheet HSSC II (If Applicable)
3. Applicant CNIC/ Form B
4. Father/ Guardian CNIC
5. Salary slip / Income certificate *
6. Domicile Certificate (Applicant or Father)

Note: An income certificate is a document that shows monthly/annual income. If;
your parents/ guardian is salaried, an original copy of a payslip should be attached, otherwise
if your parents/ guardian owns a business they should submit an undertaking on a stamp paper/affidavit (of Rs.100) stating their income and with relevant details.
