This video is designed to encourage conversations about line of fire hazards in relation to vehicles and mobile equipment. Review the video at your workplace and discuss how these types of incidents could occur where you work, what safeguards are in place, and if those safeguards are enough.

Questions for the Group:
• In our company, what situations are considered “in the line of fire”?
• The energy wheel is a useful tool to increase hazard recognition.
• Using the wheel, what other line of fire hazards could exist with vehicles and mobile equipment?
• What safeguards (i.e., controls or barriers) are used to manage these hazards in our workplace?
• Do our safeguards have capacity to fail safely if an error or mistake is made?
• Can you think of an example of complexity in our workplace that could lead to a similar incident?
• What potentially serious incidents (PSIs) involving line of fire and vehicles have we had?
