This video is designed to encourage conversations about energy-based hazards and potential blind spots prior to starting a job. Please review the video at your workplace and try to identify hazards that may have been missed during the initial pre-job assessment and discuss whether the safeguards in place are enough.

Ask your group the following questions:

Have we assessed all the potential energy sources from the Energy Wheel for this job? What ones did we miss and why did we miss them?

Where does active or stored energy exist in our workplace that could cause a serious injury or fatality?

What safeguards (controls or barriers) are used to manage these hazards in our workplace? Can one safeguard address multiple energy hazards from the Wheel?

Do our safeguards provide the capacity to fail safely if an error or mistake is made?

How could complexity lead to similar incidents in our workplace?

If you initiate a step in a critical process and it doesn’t go as intended, will the outcome be intolerable, immediate and irreversible?

What assurance activities are conducted to ensure the proper safeguards are in place?
