FAQ:
You got hit at X:XX, what gives?
There is no collision between the corners of the player (the red square) and the blue dots. This is why you often see a lot of close calls and why I regularly cut it close by running the corner of the player into the dots to buy myself just that extra bit of time. To check collision between a blue dot and the player, the game performs a check between the pixels of a blue dot and 5 different points on the player. These 5 points are the center of the player and the middle of its individual edges. When blue dots are moving fast, the collisions can miss even if it should register as a hit. This is because the game checks collision against a dot's current position and its next future position. See infographic for details https://i.imgur.com/nKz7dbc.jpg . This is most evident on level 7. On level 7 you clearly got hit by a blue dot, multiple times, why didn't you die?
When the blue dots are moving fast, the collisions tend to miss when they really should hit. Detailed explanation and diagram can be found in the above answer. Fortunately, these "missed" collisions can be consistently performed in a number of safe spots. For example, if you were to go into level 7 (either get there normally or use the level select) and press only left / right, you will never be hit by any of the blue dots even though they pass straight over you. There is another safe spot which you see me use in the run. This spot is just above the bottom wall. When I'm in this position, I can freely run across the bottom of the level without fear of being hit by a dot.

How did you grab the coin but not die on level 16?
The collision is checked in different ways for a coin and a blue dot. The blue dot does not check against the corner of the player (more information in answer 1), however the coin will collide with the corner of the player. For collision between a coin and a player, the bounding box of these two objects is used rather than a set of points. A bounding box is a box (in this case a square) which is the smallest size possible to enclose an object. Because of this, the hitbox on a coin is actually a square shape. I hit the corner of this square with the corner of the player, and as there is no collision on the corner of the player with a blue dot, I grab the coin and avoid a death.

You clipped a little bit out of a wall, how did you do that?
Wall collisions are similar to blue dot collisions (more information in answer 1) in that they do not check against the corner of the player. This allows you to place the corner of the player into a wall, and if you continue holding the correct direction you will stay partially clipped through the wall.

Where's the sound?
The speedrun version of the game has the background music and level complete sound removed. The background music was removed due to being repetitive, and the level complete sound was removed as it was buggy and played multiple times when using other features of the speedrun version. Despite the lack of music, the speedrun version is true to the original gameplay. See the below answer for a more information regarding this version.

Why does your game say speedrun?
This is a slightly modified version of the original Coolmath version of the game. The modifications add an in-game timer (the green timers in the top left of the game window), changed some text (subheader on the main menu, and in the lower right corner of the game), hot keys for restarting levels (not used during the run, practicing purposes only), and remove some game audio. You are free to download this version from the speedrun.com World's Hardest Game resources section found here: https://www.speedrun.com/whg1/resources . The original gameplay, speed of the player, collision detection, etc, remains untouched and is identical to the original version of the game.

What are those green timers in the top left of the game window?
Those are the in-game timers that were added in the speedrun version (more information above). There is a timer for the total run, and the current level. These timers are in the format MINUTES:SECONDS.FRAMES. The game runs at 30 frames per second.

Can you improve this run?
