A lactobezoar is a type of bezoar (a mass of indigestible material) found in the gastrointestinal tract of infants, usually composed of inspissated (thickened) milk curds.

Key Features
• Population: Almost exclusively seen in neonates and young infants, especially preterm babies.
• Composition: Mainly curdled milk and mucus, forming a firm mass in the stomach or intestine.
• Pathophysiology: Results from the precipitation of casein and fat in the gut, often when milk is too concentrated, high in caloric density, or when the infant’s immature digestive system cannot handle it well.

Risk Factors
• Prematurity (immature digestion and motility).
• Feeding with concentrated or high-calorie formulas.
• Dehydration.
• Reduced gastric emptying.

Clinical Presentation
• Abdominal distension.
• Feeding intolerance or vomiting.
• Palpable abdominal mass (sometimes).
• Signs of obstruction (rare, but possible).

Diagnosis
• Abdominal X-ray or ultrasound may show a mass with mottled gas pattern.
• Sometimes confirmed during endoscopy or surgery if obstruction occurs.

Management
• Conservative treatment: Stopping feeds temporarily, gastric lavage, and IV fluids often resolve the condition.
• Rarely: Endoscopic or surgical removal if obstruction is severe or persistent.
