Singapore Airlines 006 was a commercial passenger flight from Singapore to Los Angeles, California with a stopover in Taoyuan, Taiwan on October 31st, 2000 carrying 179 people. The plane was a 747-400 registered 9V-SPK.

Crash:

The plane was cleared to take off from runway 5L at Taoyuan but the aircraft instead taxied to runway 5R which was undergoing repairs at the time, at 11:16 PM the plane attempted to take off from runway 5R but ended up hitting resurfacing equipment causing the landing gear to collapse and the fuel tanks to explode. The wings detached and the fuselage broke up into 2 pieces killing 83 people with 96 survivors.

Investigation:

Because it was dark and raining out the crew couldn't see well, they neglected to review their taxi route which caused them to enter the wrong runway, the crew also neglected to check their Primary Flight Display (PFD) or their Para Visual Display (PVD) which would've indicated they were on the wrong runway.


Alitalia 404 was a commercial passenger flight from Milan, Italy to Zurich, Switzerland on November 14th, 1990 carrying 46 people. The plane was a Dc-9 registered I-ATJA.

Crash:

The plane was nearing Zurich for an ILS landing in the middle of the night when the First Officer felt uneasy about their approach so he began a go-around, the Captain stopped him from doing do but just seconds later the plane started hitting trees, the right wing was sheared off and the plane inverted and crashed into the Stadlerberg Mountain killing everyone on board.

Investigation:

Due to a malfunction in the plane's receiver, the ILS displayed the plane was 1,000 feet higher than it was, this didn't affect the First Officers display so he tried to attempt a go-around but the Captain stopped him and they weren't able to avoid a collision with the mountain.


TransAsia Airways 222 was a commercial passenger flight from Kaohsiung, Taiwan to Huxi, Taiwan on July 23rd, 2014 carrying 58 people. The plane was an ATR-72-500 registered B-22810.

Crash:

While approaching Huxi in dense fog the pilots were cleared to descend to 400 feet, the plane descended to 72 feet before the pilots tried to make a missed approach but struck trees and crashed into Xixi village killing 48 of the 58 people on board.

Investigation:

The Captain was extremely overconfident and cocky and instead of descending to 400 feet he descended to 300 feet which were below the minimum safe altitude, he then descended to 200 feet and attempted to locate the runway but unbeknownst to him the plane had made a left turn throwing them off course, he then descended more and crashed.


Korean Air Cargo 8509 was a scheduled cargo flight from Seoul, South Korea to Milan, Italy with stopovers in Tashkent, Uzbekistan, and London, England on December 22nd, 1999 operated by 4 crew members.

Crash:

After the plane had departed London the Captain tried to turn left but his Attitude indicator showed the plane was level, the plane banked more and more left before the plane crashed into trees 1.5 miles from the airport killing everyone on board.

Investigation:

The Captains Attitude indicator had malfunctioned from an incorrect INS system, the First Officer's Attitude Indicator wasn't malfunctioning and was working well but the First Officer was unwilling the challenge the more experienced Captain leaving the plane to undetectably roll into the ground.


United 585 was a commercial passenger flight from Peoria, Illinois to Colorado Springs, Colorado with stopovers in Quad Cities, Illinois, and Denver, Colorado on March 3rd, 1991 carrying 25 people.

Crash:

At 9:37 AM the plane was cleared to land at Colorado Springs when it suddenly rolled right, it then spiralled to the right and dove straight into the ground killing everyone on board.

Investigation:

An unknown issue with the 737's rudder occurred when hot hydraulic fluid flowed into cold rudder parts causing thermal shock which made the rudder to lock up and hard over, USAir 427 was also a victim of this, the issue was eventually fixed.


Inex-Adria Aviopromet 1308 was a commercial passenger flight from Ljublana, Yugoslavia to Ajjacio, France on December 1st, 1981 carrying 180 people. The plane was an MD-81 registered YU-ANA.

Crash:

The plane was descending to Ajjacio in dense clouds when the pilots saw Mont San-Pietro in front of them, the pilots began to climb but the left wing of the plane struck the mountain and the aircraft dived down into terrain on the other side of the mountain killing everyone on board.

Investigation:

The ATC believed the plane was out of it's holding pattern when it was really in it thus the ATC told the plane to descend, this was the Inex-Adria's first flight to France so the crew were inexperienced which made them believe the ATC so they descended. The airport had no radar system and the clouds in the area obscured vision, the ATC's imprecise phrasing was also a factor in the crash.
