Dr. Corenman discusses Neurological and Non-Degenerative Spinal Disorders and Diseases. His favorite quote by Osler is “You see what you look for and find what you know.” If you are not aware that these disorders occur, you will never think about them and won’t know what to do.

The first one is quite common unfortunately: neuropathic pain. This is pain generation without current nerve or tissue injury. You have chronic pain signals delivered like a doorbell with a short circuit but no tissue damage. This occurs because the nerve itself can become crushed, stretched, inflamed or a have a vascular injury to the nerve. A “leaky” nerve membrane or a sodium/potassium pump failure may occur. The patient may experience a burning pain, constriction (“tight band), allodynia (pain generated from innocuous stimulation (touch or pressure) is common, and hyperalgesia. The nerve damage pain signals are relentless and become amplified known as the “rock in the sock. This is worse at night by can be affected by activity.

One easy way to remember which abdominopelvic organs are retroperitoneal is to use a mnemonic such as SAD PUCKER:
S = Suprarenal (adrenal) glands.
A = Aortal/Inferior Vena Cava
D = Duodenum (second and third segments)
P = Pancreas
U = Ureters
C = Colin (ascending and descending only)
K = Kidneys
E – Esophagus

The other topics highlighted include: referral and cyclical patterns of various retroperitoneal organs, history/physical examination of neurological disorders, myelopathy, multiple sclerosis, polio, Tabes Dorsalis, amyotrophic lateral sclerosis, Parkinson’s disease, Charcot Marie tooth disease, Lyme disease, Guillain Barre Syndrome, Herpes Zoster (Shingles), Parsonage-Turner Syndrome, Diffuse Idiopathic Skeletal Hyperostosis (DISH), Autoimmune Vertebral Involvement (Spondyloarthropathies), Unilateral SI Involvement (Seronegative Spondyloarthropathies), Ankylosing Spondylitis and physical exam findings,, Ankylosing Spondylitis II, Reiter’s Syndrome, Psoriatic Arthritis, Enteropathic Arthritis, and Rheumatoid Arthritis.

Dr. Corenman talks about Polyneuropathy (peripheral neuropathy). This is length dependent: nerve of equal length are equally involved (symmetric). Sensory loss equal in both feet and ascend up the legs equally. By the time the numbness gets to the knees, the bilateral hands become involved (fiber-length dependent). There are also metabolic peripheral neuropathies.

Lastly, Dr. Coreman touches on RSD/CRPS (Complex Regional Pain Syndrome) of which there is no cure. He closes on myopathies, fibromayalgia and medico-legal considerations.
