SETTING: Home Visit

NURSE: You are visiting an 80-year-old patient who underwent hip replacement surgery three weeks ago. You check on his/her progress and address his/her concerns.

TASKS

* Find out how the patient is feeling. ( pain or discomfort)
* Explore possible reasons for the patient's poor sleep (anxiety, stress, pain, etc.).
* Find out more about the patient’s symptoms (e.g., the severity of the headache, associated symptoms, treatment tried, etc.).
* Explain symptoms that suggest acute sinusitis (inflammation of the sinus lining at the back of the nose, causing facial pain, fever, runny nose, etc).
* Reassure the patient (e.g., the condition is usually self-resolving, not serious, etc.).
* Recommend self-help measures (e.g., high fluid intake to loosen secretions. gentle nose-blowing, head elevation when lying down, etc.).
* Stress needs to see GP only if there is no improvement in one week (e.g., GP to prescribe appropriate treatment, rule out other conditions, etc.). Stress likely effectiveness of self-help treatments.
* Advice on when to seek medical help ( if there is no improvement in symptoms or symptoms worsen). Reassure the patient that the doctor will prescribe medications like decongestants and antibiotics, etc).
