Updates on the recovery of a patient six months after a major skin excision gynecomastia procedure with free nipple grafts by Dr. Caridi of the Austin Gynecomastia Center.

This wonderful patient had been seen in the office on several occasions for consideration of treatment of significant bilateral gynecomastia. He lost a significant amount of weight over time and still needs to lose weight. In terms of the treatment of gynecomastia, no doubt, we feel he was ideal for excision of soft tissue of the chest and subaxillary region with a free nipple graft transposition technique. The risks of this procedure include, but are not limited to bleeding, infection, hematoma formation, scar deformity, contour deformity, pain, paresthesias, numbness, asymmetries, and dissatisfaction with results. We addressed that significant scarification can occur in the results of this procedure with loss of nipple sensation from the nipple areolar grafting techniques. We also spoke of the hypopigmentation that can exist after the free nipple grafting technique which was evident in his six month post-operative appointment. Hypopigmentation refers to patches of skin that are lighter than the overall skin tone. This is caused by the loss of pigment cells in the nipple areola complex grafting techniques. This patient used silicone scar strips to improve healing with great success.

What was interesting was that this patient LOVED the hypopigmentation of his nipple areola complex and has no inclination for nipple tattoo or treatment. This was something Dr. Caridi was intrigued and impressed with. It goes to show that the positive effects of removing all of the extra skin of the chest and liberating this man of the physical and psychological aspects of having gynecomastia far outweighs the hypopigmentation of the nipples. In fact, he seems quite fond of his unique recovery.
