Metal Gear Solid 3 - How to Escape Groznyj Grad on Extreme with No Alerts! (guide in description)

This footage is from my recent MGS3 European Extreme difficulty Foxhound ranking run. This strategy may take quite a few tries to get down, but this will help if you're having trouble getting through these areas in particular, and also if you're aiming to get that elusive Foxhound ranking.

When you're in the cell, remember to throw the food back at the guard 3 times, which triggers the cutscene where he tells you his name is Johnny. This is because he also gives you the Cig Gas Spray in this cutscene, which you'll need for this section.

After the cutscene, knock on the cell door and wait for Johnny to come over. Put him to sleep with the Cig Gas Spray, and then ring the frequency 144.75 to unlock the cell door. Remember to drag and drop Johnny twice; the first time you get Smoke Grenades, which you'll also need for this section, and the second time you get Instant Noodles.

Before leaving this area, make sure to equip the Smoke Grenades, and make sure to eat the Instant Noodles to restore your stamina. Remember to also remove the fake death pill and the transmitter; if you don't, it will become much harder to remain undetected in the upcoming waterfall section. I would also advise you to switch to the original Snake Eater camera for the next 2 areas by pressing R3, as it helps with orientation.

In the first area, run up to the edge of the wall, press against it, and wait for the guard to turn around. Once he does, cook a grenade and run with it to silence your footsteps. Run straight downwards all the way to the wall, and then run flat against the wall almost all the way to the left. You can unequip the grenade here.

Run diagonally towards the big metal box and wait for the guard who saw you to approach. Once he stops looking in your direction and walks past, roll or run to the vent in the upper left corner, then crawl through the vent and into the next area.

In the second area, just run all the way to the door on the left side as flat to the bottom wall as you can. Guards will see you in the distance, but they will not raise an alert as long as you don't stop running. Run through the door into the next area.

In the third area, switch back to the Subsistence camera by pressing R3, equip the Smoke Grenades, then run up to the second tank and wait for the guard to turn around. When he does, cook a grenade and run with it, then when you get behind him, switch to the Cig Gas Spray and put him to sleep.

Sneak all the way up to the last tank, wait for the dog to move, and for the enemy on the right to look away. When they have done this, cook a grenade and run up to the guard by the truck, then switch to the Cig Gas Spray and put him to sleep.

Now equip the Smoke Grenades and run towards the guard and the dog. Throw all 3 Smoke Grenades, making sure you hold the button down for a bit each time so you throw them further. Aim the first one at the guard and the dog, aim the second one between them and the door, and aim the last one at the door.

By the time you've thrown the last Smoke Grenade, there will be a Caution state raised, but it won't be a problem as the guard and the dog won't be able to detect you due to the smoke. Immediately run to the door, then crawl underneath the pipe, and you will make it to the sewers.
