After losing 100 pounds, this patient was ready for gynecomastia surgery. Loose skin after massive weight loss presents an interesting challenge – is loose skin removal surgery needed or will the patient’s skin elasticity be enough to achieve the chest he wants? A natural, masculine looking chest does not include scars, so we explore whether a standard treatment of Vaser liposuction and glandular tissue removal will be enough or if skin removal is needed.

Struggling with gynecomastia since he was 12 and after losing 100 pounds after weighing 265 pounds in 2017, the patient was finally ready to finish his transformation into his new self. Around when starting to lose weight, he realized that the only way to treat gyno was with surgery – a fact that countless others with gynecomastia eventually discover for themselves. After doing his research, our patient decided to entrust his gyno removal and possible excess skin removal surgery with our Austin gynecomastia surgeon, Dr. Robert Caridi at the Austin Gynecomastia Center. We begin as Dr. C and the patient review their surgical strategy, having agreed to proceed in a “experiment” to see if the patient will need to have skin excised (removed) or if his skin’s elasticity will be able to forgo skin excision and the resulting scarring from it. With over 30 years of cosmetic surgery experience, Dr. C has become an expert in determining whether a patient’s skin elasticity will be able to achieve the results the patient is looking for, but in this case, he will push the limits of that expertise to see just how good our patient’s skin is.

As with any major loss of weight, there is excess, loose skin, and the removal of glandular tissue and fat with gynecomastia surgery produces even more excess skin. The question of whether this excess skin should be removed by surgery or if the skin’s elasticity (the skin’s ability to stretch and snap back to its original shape) should be considered in these cases. All too often, skin is removed without considering whether the skin will be able to retract all by itself, resulting in scarring that could have been avoided. Skin removal and the resulting scars are not reversible, so this decision should not be rushed into and avoided when possible.

Knowing that he was in great hands, the patient knew that Dr. Caridi would deliver the results he had been dreaming of for so long. When agreeing to undergo this “experiment”, both Dr. C and the patient knew that if it did not turn out they way that they wished it would, the loose skin could be removed in a follow-up procedure.

In this video, we cover the patient’s gynecomastia removal surgery and his follow-up visits at 2 weeks post-op and 2 months post-op, learning just how well Dr. C’s expectations hold up. Removal is a standard gynecomastia treatment of Vaser liposuction and complete glandular tissue excision. With the assistance of internal quilting stitches and internal flaps, the skin and tissue are redistributed and secured in a way to avoid depressions and to pin up the chest so that it will allow the natural healing process to produce a natural, masculine appearance. Afterwards, find out just how well the patient is looking and the results of how well his skin performed, as well as what we all can learn from the generosity of this amazing young man.
