ACL injury is classically treated with ACL reconstruction surgery. Nowadays a recent advancement is to repair ACL ligament in selected group of patients. ACL repair is a technically difficult surgery performed at selected centers.
Why is ACL Repair a better surgery :
Historically ACL Repair is infamous for poor results. Because technique was old . Open surgery was done. Knee was immobilised.
Now a days standard of care in ACL surgery is ACL reconstruction.(Basically remove old and put New). There are mechanoreceptors present on the ACL stump and if we remove them than proprioception or balance is hampered. This is the common scenario in ACL reconstruction surgery This is a very good surgery with excellent results but it has got its limitations.
1)Decreased Proprioception [ there are balance receptors (NerveEndings)in the ACL stump and if you remove them than balance perception will reduce]
2) Slightly more recovery time, as body has to adjust to the reconstructed ligament.
3) Grafts need to be used (donor side problems)

But now there is recent technical advances in ACL Repair . ACL repair is an entirely different surgery than ACL reconstruction .ACL repair work on principle of repairing the stump back to its own position. IN ACL repair Surgery, the scenario is different as it is a constructive surgery in which the native ACL is repaired back to its normal location. The nerve endings are preserved and better proprioception will be there. This leads to a faster recovery and return to sports
ACL repair surgery can be performed by 2 methods:
1)Knotless Anchors : Like Swivelock Here the sutures are passed through the ACL stump and the sutures are threaded in the eyelet of the knotless anchor to attach the ACL stump back to the footprint.
2) Suture Button technique, Here serial sutures are passed through the ACL stump and then a button along with threads are looped .
The benefits of ACL repair over ACL Reconstruction are :
1) Faster Recovery (Quick Recovery)
2) Avoidance of graft morbidity (No graft needed hence less traumatic to knee)
3) Better Proprioception (More Confident knee )
4) Less Pain
5) Less Swelling
6) Less Quadriceps atrophy
7) No Large Bone Tunnels drilled
8) Less Infection rate as Grafts not used

But there are some limitations to this technique as well
1) Not possible in majority of cases due to absense of a good stump to repair
2) Retears are known complication
3) can't be performed in old chronic tears

Usually a Internal Brace Technique is added in these cases to protect the Repair.
