CHOLESTEATOMA
If you have a Cholesteatoma in the ear the only treatment is surgery.
Waitful watching is largely for a few selected patients who are unfit for surgery.
WHAT IS A CHOLESTEATOMA?
A Cholesteatoma , by definition, is skin in the wrong place.
Due to various processes including prolonged or recurrent negative pressure, migration, implantation the external auditory canal skin migrates into the inner ear and causes infection and complications related to that.
Cholesteatoma is aggressive in nature and causes destruction of the surrounding bone, specially in an anaerobic environment.
Since the ear contains and is surrounded by very delicate and important structures, a Cholesteatoma can invade and destroy them
This includes the Tympanic Membrane, the ear ossicles, the thin membranes between the middle ear and the inner ear, the bony partition between the ear and the brain , the facial nerve etc.
As we can see a Cholesteatoma can cause serious and sometimes life threatening complications.
It can also cause a very debilitating paralysis of facial muscles on account of damage to the Facial Nerve.
The only treatment for a Cholesteatoma is surgery.
The surgery performed is a Mastoidectomy which is usually under General Anaesthesia.
WHAT IS MASTOIDECTOMY?
A Mastoidectomy is a surgery on the mastoid bone which is a part of the Temporal Bone- The temporal bone houses the ear.
In Mastoidectomy the mastoid bone is drilled in order to clear disease, create access, make the ear safe from dangerous diseases like Cholesteatoma, and give access for other surgeries including introducing a Cochlear Implant.

WHAT ARE THE TYPES OF MASTOIDECTOMIES
There are a number of Mastoidectomies, but they can be broadly classified under
• CWU- Canal wall Up mastoidectomy
• CWD – Canal wall down – the posterior canal wall is taken down.
There are specific indications of each including the nature and extent of the disease, primary disease or recurrence, personal choice of the surgeon etc.
The CWD technique leads to the formation of a mastoid cavity which requires regular maintenance.
