In this video, Dr. Mark Mason, board certified plastic surgeon with 20 years of experience, explains how and where breast implants are placed during breast augmentation or breast implant exchange surgery. He also explains options for breast implant placement over or under the pectoral (chest) muscle.

There are several options for incisions for breast implants. I prefer the Inframammary Crease Incision. This incision allows for several placement types and both insertion and removal of the implant. The incision is made in the crease under the breast, allowing for discreet scarring and has the lowest rate of infection. Other incision areas are the lower areola and the armpit.

Another decision that needs be made is weather to place the implant on top of, or under the pectoral (chest) muscles. If a patient has significant soft tissue coverage then it is best to place the implant on top of the muscle, because naturally the breast rests on top of the chest muscle.

Many women that come in for breast augmentation do not have a large amount of tissue on top of the muscle. In this case it is best to place the implant under the muscle. We say under the muscle but, the chest wall muscle only covers about one-third to one-half of the implant.
