1. Extreme fogging in the aircraft cockpit caused an F-16C Fighting Falcon (#87-0296) to depart the runway July 28, 2011, at the Wittman Regional Airport in Oshkosh, Wisc., according to an Air Combat Command Accident Investigation Board report

As they were pulling out, the pilot saw the correct letter and decided to circle around for another attempt. There were 72 seconds between the initial "no-drop" and the "green light" for the second attempt, which happens at 0:42 in the video. This is why the critical moment was "30 seconds before."During the "no-drop," the loadmasters performed the appropriate checklist, which included locking the left rail, a hard lock, while the right rail uses a tension release. When the pilot opted for a second attempt, all pre-drop checklists had to be redone at 20 minutes, 10 minutes, 5 minutes, 1 minute, and 30 seconds before the drop. The remarks "better boogie here boys" at 0:20 and "we're not going to make it" at 0:31 indicated their concern about completing the checklists in time. In the rush to redo the checklists, the step to unlock the left rail was missed. Just after the "green light," the loadmasters and pilots quickly realized something was wrong because the load did not shift as expected upon exiting the airplane.
