Kildare West Wicklow Integrated Care Programme for Older Persons (KWW ICPOP)

The Kildare West Wicklow Integrated Care Programme for Older Persons (KWW ICPOP) team is based in St. Mary’s, Craddockstown Road, Naas, Co. Kildare and is part of the wider Dublin South Kildare Wet Wicklow Older Persons Service.

The team is aligned to the National Clinical Programme for Older Persons and supports people over the age of 65 who live in the Kildare West Wicklow area. We are aligned to Naas General Hospital . We are a multidisciplinary team that works across the acute and community healthcare settings provide care to the older person and their families. Our community based services allows the older person to access integrated care and support that is planned around their needs and choices, enabling them to live well in their own homes and communities.

Our aim is to support older people to remain at home as well as possible, for as long as possible, by creating and augmenting links between acute and community service.
