The Integrated Care Programme for Older Persons (ICPOP) team is based in Cherry Orchard Hospital campus. The team looks after older persons who live in the Dublin West and Dublin South City area. We are aligned to St. James University Hospital. We are a multidisciplinary team that works across the acute and community healthcare settings to wrap around the older person and their families. Our community based services allows the older person to access integrated care and support that is planned around their needs and choices, enabling them to live well in their own homes and communities.

The team is ideally placed to deliver on Sláintecare goals of the right person getting the right treatment in the right place at the right time. It does this by identifying patients in the community that are at high risk of a crisis ED attendance as a result of living with dementia and frailty and we proactively manage these patients from their home, optimising their medical problems, medications, function and social supports using a comprehensive geriatric assessment framework. Our aim is to support them to remain at home safely. We’ve managed over 500 patients and are successful in preventing ED attendances, hospital admissions and significantly reducing carer stress, enabling families to continue to support their loved one at home for longer.

Our integrated approach to care with the acute hospital means that we can also offer early supported discharge from hospital. For the right patient it is a safe a preferable way to provide whatever they needs have, be it medical, rehab or care needs in their own home. With this approach, we have been successful at providing domiciliary rehab to patients following hip fracture, for example, getting people home on average 37 days sooner than if they had traditional inpatient rehab.
Feedback from patients has been overwhelmingly positive and they enjoy the collaboration with the team in delivering their care.
