Does you engine sound like this it the morning? If it does, you might need to look at your valve adjustments. Detroit recommends to have your valves adjusted at 100K miles and then at every 500K miles.

I definitely do not agree with this, I recommend to have your valves looked at or have a compression test done every 100K to 150K miles as part of your regular maintenance. It is a lot less expensive to have your valves adjusted than to have your cylinder head replaced or possible engine failure because of a dropped valve that can wipe out an engine.

These newer emissions engines are not like the old school Detroit Series 60, Cummins N14 or CAT engines. I know it’s not what we want to hear but it’s the times we live in.
