In this video we discuss the exact meaning of SOAP notes, also known as SOAP charting.

Subjective is anything that client tells the therapist.
Objective is anything the therapist observes.
Assessment/Application is anything that took place during the treatment.
Planning is the plans going forward for treatments.

This video also explains the Signs and Characteristics of Skin Cancer using the Acronym ABCDE, make sure you know this before going into the MBLEx.

If you are student preparing for the MBLEx, make sure you know this information, remember that client assessment, reassessment, and treatment planning is worth 17 percent of the MBLEx. Meaning there is likely going to be 17 out of 100 questions that may cover materials like this.
