In today’s video, we are joined by Lacy Stewart, one of the incredible nurses here at the Austin Gynecomastia Center, and a patient who is 1 day post-op from his gynecomastia surgery with the Man Boob King, Dr. Caridi!

The focus of this short video is all about what you need to know the first day after surgery.

When it comes time to take off your garment, you do want to be sure that you are lying flat. Underneath the garment, you will have all the padding that was placed after surgery. Saturation is normal, and you should expect to see fluid all over the padding. The lipo foam is your best friend, and is used to help combat swelling and help the skin adhere back.

With a standard gynecomastia surgery, you will have your incisions under the nipple and one under the axilla. This is a good time to point out, that this is not when you should start judging the results of your surgery. Your body is traumatized and there will be some irregularities because there is a lot of fluid placed to soften up the fat for the liposuction. Know that your body is doing what it needs in order to heal, so be patient and work the process.

The first thing that we look for post-operatively is a hematoma. If there, it will be visible to the eye, whereby one side of the chest will be twice the size of the other, and it will include heavy bruising and come with significant pain.

The stitches used on the nipple-areola are absorbable and they will typically fall out on their own around the 2-week mark. If the stitches are still there after we weeks, you can take a pair of tweezers and pull them out.

At this point, patients will be able to take a shower. If you are observing a loved one, make sure this is done slowly and carefully. All position changes should be closely monitored and done slowly to ensure they feel stable before moving. When taking a shower, make sure the water isn’t super hot, and gently use soap and water over the incisions. Be sure not to scrub, just let the soapy warm water run over the treatment area.

Changing your dressings requires antibiotic ointment the first 2 days after your surgery. You will receive ointment in your take-home kit, but any antibiotic ointment will do. The reason for only using ointment the first two days after surgery is because you want your body to naturally heal and adding too much can keep your incisions overly moist preventing your body from doing what it wants to do on its own. Additionally, we have started telling patients that you can use a drop of hydrogen peroxide on the end of a Qtip to help get rid of the scabs.

When placing ABD pads, they should go over the incisions in the axilla and over the chest.

Continue to wear your lipo foam for at least 2 weeks after surgery, and ideally even longer if you have significant amounts of excess skin (we will let you know if you are one of those guys). If you have excess skin, you want to wear the lipo foam for at least 4 weeks. Placing the lipo foam is easy, simply place it over the treated area, and it will be held in place with the garment and ace wrap. That ace wrap is only required for the first 2 days after surgery and then you no longer need to wear it!

Nurse Lacy has a super clever trick for how to wrap the Ace wrap if you are all by yourself. The Ace wrap should be snug but not so tight that you can’t breathe.
