Checklist for Confined Space:
1. Surrounding area checked and all combustibles within 35 ft are removed and protected (Yes / No)
2. Inlet/Outlet lines disconnected and isolated (Yes / No)
3. Vent, Manholes are kept open and proper access to and exit from work place provided (Yes / No)
4. Inside temperature is at the room temperature (Yes / No)
5. Portable tools / 24 V hand lamp properly earthed / double insulated / low voltage (Yes / No)
6. Safety observer placed at the entry into the confined space (Yes / No)
7. Enough oxygen in ambience (especially hot work in confined space )safe level 19.5 to 23.5 % (Yes / No)
8. Gas testing performed to detect the presence of toxic flammable gasses (Flammable gas - less than 5% of lower explosive limit, Hydrogen sulphide 10 ppm, Carbon mono-oxide 30 ppm. (Yes/No)

The Confined Space Entry Permit is the most essential tool for assuring safety during entry in confined spaces with known hazards or with unknown or potentially hazardous atmospheres. The entry permit process guides the supervisor and workers through a systematic evaluation of the space to be entered.

OSHA's standard for confined spaces (29 CFR 1910.146) contains the requirements for practices and procedures to protect employ-ees in general industry from the hazards of entering permit spaces. Employers in general industry must evaluate their workplaces to determine if spaces are permit spaces.

Rescuer Tragedies. OSHA dictates that the minimum "safe level" of oxygen in a confined space is 19.5%, while the maximum "safe level" of oxygen in a confined space is 23.5%. With low oxygen levels being the biggest cause of death in confined spaces, accurate oxygen level measurements are essential.
