1. Area isolated by operating molded case circuit breaker (MCCB) or Air circuit breaker (ACB)
2. Isolated and locked out Vacuum Circuit breakers (VCB) or Load break switches (LB switch) (Yes/No)
3. Concerned Fused Removed (Yes / No)
4. Rubber mats provided (Yes / No)
5. All possible back up feeding supplies disconnected (Yes / No)
6. Equipment Earthed (Yes / No)
7. Line / Equipment area barricaded (Yes / No)
8. Discharging of capacitors done (If required) (Yes / No)
9. Electrical gloves inspected and provided (Yes / No)
10 Department & production authorities informed(Yes / No)
