Pectoralis Minor (Pec Minor) Anatomy
Triangular shaped. Deep to pectoralis major.
Origin: Third, fourth and fifth ribs close to respective costal cartilages.
Insertion: Medial border and upper surface of coracoid process of scapula.
Concentric Actions: Scapular protraction and shoulder depression.
Accessory breathing muscle: Elevation of ribs.
Innervation: Medial Pectoral Nerve (C8, T1).

Symptoms
Pain with active contraction of pectoralis minor.
Pain described as dull, sore achy and tight.
Tenderness upon palpation of medial coracoid process of scapula.
Tightness in pectoralis minor with scapular retraction.
Positive Bench Press Maneuver.

Individuals with shorter resting pectoralis minor muscle length have incorrect scapular movement patterns similar to those reported in individuals with shoulder pain secondary to subacromial impingement syndrome, rotator cuff disease and glenohumeral instability.

Tight pectoralis minor muscle creates a position of scapular protraction at rest and does not allow scapular posterior tilt or external rotation upon arm motion, predisposing patients to subacromial impingement symptoms.

Manual Length Test: Pectoralis Minor
Supine. Hands resting on stomach or by sides.
Relax shoulders and let posterior shoulders drop toward table.
Positive: Scapulae stays protracted and shoulder is lifted of table.
Indicates: Hypertonicity or shortening of pectoralis minor.

Bench Press Maneuver
Supine. Simulate bench press against resistance.
Positive: Pain in Pectoralis Minor
Indicates: Pec Minor Overuse Insertional Tendinopathy AKA Bench‐Presser's Shoulder
